What are 'Living My Life' services?

“Living my life” is a disability support service for people who have had an injury that has led to a long-term disability, which affects their ability to participate and engage in everyday activities.

Our team’s involvement can empower you to make choices about your life and achieve your personal, community and employment goals.

We can support you to find ways to take control of your journey, by reducing barriers and promoting your independence, so you can live a fulfilling life.

What is the goal of 'Living My Life'?

The name of the service says it all – how we want to live our lives, including with injury-related disability, will be unique to each of us. “Living my life” aims to give you, or a loved one, more choice and control over the important decisions in your life, such as:

  • Where you want to live
  • What you want to do for work or activities in your community
  • What your long-term goals are
  • Who you choose to support you

Who delivers the service?

Explore’s interdisciplinary team supports you in your local community through input from a range of professionals, tailored to your needs, such as from:

  • support workers 
  • occupational therapists 
  • physiotherapists 
  • registered nurses 
  • social workers 
  • speech language therapists. 

How is the service delivered?

Living My Life services are provided in four parts that can be used at different times and stages in your life.

  1. Facilitated pathway map 

You will be supported to create a visual map about what you want to achieve. The map describes your goals, and the people and places that are important to you.

  1. Independent facilitation 

You will be empowered to make decisions about how you want to engage in your community, what supports you may need, and where to find them.

  1. Tailored support

We can support you one-on-one and in groups to:

  • Do meaningful activities that you enjoy 
  • Explore your interests, try new activities, and build relationships 
  • Find ways to live where you feel at home
  • Find or stay in paid or unpaid work  
  • Develop your independence by learning new skills

Our Explore team provides this service nationwide.

How much does it cost?

Fully funded service*

Our living my life programme is fully funded by ACC for people who are eligible. This means that you don’t have to pay to use our services, within the limits of your funding.

To check if you’re eligible for our fully funded services, speak to your ACC case manager.

 

Privately paid service

Alternatively, if you aren’t eligible for fully funded support, you can pay for our service using your own money. Talk with our team to discuss pricing and support options.

When can I start?

Once ACC has approved your funding, you can start working with us straight away. There’s no waitlist for this service.

 

How long does the service last?

Living my life services continue until you no longer need support to manage your own activities and engage in everyday life.

How accessible are your services?

Every disability or health condition is different, that’s why you’ll receive one-on-one support that meets your needs and preferences. No matter what barriers you face, we can work with you to make sure you can access our services.

How do I refer myself or someone else?
